The Durable Power of Attorney for Health Care is a legal declaration that may be recorded in the Recorder's Office once the proper statutory requirements are met.

There is a fee of $28 for the first two pages and $8 for each additional page to have your Durable Power of Attorney for Health Care filed at the Recorder's Office.
